1. Scope of Services

Metta Wellness NW provides telehealth consultations, functional medicine guidance, and wellness-related services. Our services are intended for non-emergency medical consultations and should not be used as a substitute for in-person medical care when required.

2. Eligibility

To use our services, you must:

• Be at least 18 years of age or have legal guardian consent.

• Reside in a state where we are licensed to provide telehealth services (Washington, Oregon, Colorado, Idaho)

• Provide accurate and truthful medical history and information during consultations.

 

3. Medical Disclaimer

​

• Our telehealth services do not replace traditional in-person medical care and are not intended for medical emergencies. If you experience a medical emergency, call 911 immediately.

• We do not prescribe controlled substances through telemedicine except in certain cases testosterone replacement therapy.

• Recommendations made during consultations are based on the information provided by the patient and do not constitute a guaranteed diagnosis or treatment outcome.

 

4. Appointments & Cancellations

• Patients must schedule appointments in advance through our website or telehealth platform.

• Cancellations or rescheduling should be made at least 24 hours in advance. Late cancellations or missed appointments may be subject to fees.

 

5. Payment & Refund Policy

​

• Our services operate on a cash-pay model. Payment is required before the appointment unless otherwise stated.

• We accept HSA/FSA payments but do not directly bill insurance.

• Refunds are not provided for completed telehealth services. If you believe you were charged in error, please contact our support team.

 

6. Privacy & Security

​

• We are committed to protecting your personal and medical information. All data is stored securely and in compliance with HIPAA regulations.

• We use secure telehealth platforms to conduct appointments, but we are not responsible for data breaches caused by third-party providers.

7. SMS/Text Messaging Terms

By providing your mobile number, you agree to receive text messages from Metta Wellness NW PLLC related to your account, services, appointment notifications, or promotional offers.

Message frequency may vary. Message and data rates may apply depending on your mobile service provider.

You can cancel the SMS service at any time. Just text “STOP” to opt out. After you send the “STOP” message, you will no longer receive text messages from us. You can also contact us at support@mettawellnessnw.com to opt out.

If you experience issues, reply “HELP” for assistance or contact us directly.

Metta Wellness NW PLLC is not liable for delayed or undelivered messages. You understand that text messages are not secure and may be intercepted. You assume all responsibility for any data transmitted via text messaging.

 

8. Patient Responsibilities

By using our services, you agree to:
